The ED holds a monthly Morbidity and Mortality Meeting. The day alternates between nursing and medical education time each month and is always advertised on the ED Central calendar. Everyone is encouraged to attend and the more people there, the more discussion and productive it is.
This is a chance to look at system issues within our department that might contribute to deaths, unnecessary morbidity, complaints and representations. Some important initiatives have arisen from this process in the past: for example the between the flags age specific ED observation charts were in response to cases discussed in the M+M meeting.
If there is a case that you think should be reviewed in this forum then let the NUM or Director know.
